Attach a separate list if required or provide a map clearly identifying the lot/DP numbers of land to be included. The Biodiversity Values Map viewer can be used to identify the land to be reviewed.

Why do you believe the Biodiversity Values displayed on the identified land are incorrect? Please list all the Biodiversity Values you wish to be reviewed and enter the reason for review. The User Guide for the Biodiversity Map viewer explains how to identify the biodiversity value of land included on the Map (pages 12 and 13).

Please provide copies of photos or any existing reports you may have to help support your application. If you have multiple areas/categories to be reviewed you may wish to a map detailing the areas/categories of land to be reviewed.
